Woke Up With Neck Pain? Why Your Neck May Hurt After Sleeping

You felt fine when you went to bed.

Then you wake up, turn your head—and suddenly your neck feels stiff, sore or almost locked.

For some people, the discomfort improves after they start moving. For others, turning the head to back out of the driveway or looking down at a phone remains painful throughout the day.

An occasional stiff morning can happen to almost anyone. But if you regularly wake up with neck pain, there’s usually more worth considering than simply blaming your pillow.

Why Does My Neck Hurt When I Wake Up?

Sleeping places your neck in relatively fixed positions for several hours.

If your neck is already irritated, spending the night in one position can make the stiffness more noticeable when you first wake.

Potential contributors include:

  • Sleeping with the neck rotated for prolonged periods
  • A pillow that doesn’t comfortably support your preferred sleeping position
  • Muscle or joint irritation
  • Reduced cervical mobility
  • An unusually strenuous activity the previous day
  • Extended computer or phone use
  • Previous neck injuries
  • Degenerative changes in the cervical spine

The important point is that morning neck pain isn’t a diagnosis.

The pattern of symptoms helps determine what should be examined.

Is My Pillow Causing My Neck Pain?

Possibly—but buying another pillow isn’t always the solution.

A pillow primarily needs to help you sleep comfortably while accommodating your body and preferred sleeping position.

Side sleepers generally require enough support to prevent the head from dropping excessively toward the mattress. Back sleepers often need less height.

Stomach sleeping typically requires substantial neck rotation, which some people with an already sensitive neck find uncomfortable.

Before purchasing an expensive “orthopedic” pillow, ask a simpler question:

Do I consistently wake with symptoms after sleeping in a particular position?

That observation may be more useful than the pillow’s marketing claims.

Why Is My Neck Stiffest First Thing in the Morning?

Movement changes after you’ve been relatively stationary for hours.

It’s therefore not unusual for musculoskeletal discomfort to feel different when you initially get out of bed.

What matters is what happens next.

Does the stiffness disappear within several minutes?

Does it persist for hours?

Does turning one direction reproduce sharp pain?

Does discomfort travel toward your shoulder or arm?

Does this happen every morning?

Those distinctions can help determine whether the problem warrants evaluation.

Neck Pain That Travels Into the Shoulder or Arm

Pay closer attention when neck pain isn’t confined to the neck.

Pain, tingling, numbness or weakness extending into the shoulder, arm or hand may indicate neurological involvement and deserves an appropriate examination.

The exact distribution matters.

Tell your provider which fingers are affected, whether your grip feels different and which neck positions change your arm symptoms.

Should You Stretch a Stiff Neck?

Gentle movement may feel helpful for uncomplicated stiffness, but aggressively forcing a painful neck isn’t necessarily better.

Instead of trying to “crack” or force the neck through a painful restriction, start with comfortable movement.

Persistent or recurring limitation deserves an examination rather than increasingly aggressive self-treatment.

Exercise is an important component of conservative neck-pain management. A 2024 randomized trial found that two different neck-exercise approaches both improved pain, disability, posture measures and deep-neck-flexor performance in people with chronic neck pain. PubMed

What About Chiropractic Treatment?

The appropriate treatment depends on what your examination shows.

Current evidence does not suggest that every patient with neck pain should receive exactly the same treatment. Instead, clinical guidelines generally favor individualized, multimodal conservative management.

A 2025 systematic review examining international neck-pain guidelines found that all of the high-quality guidelines it identified recommended manipulation or mobilization, with or without exercise. Appropriate screening and patient selection remain important. PubMed

A randomized trial also found that manipulation combined with exercise produced greater short-term improvements in several outcomes than either approach alone for mechanical neck pain. PubMed

Consider having recurring neck pain checked when it:

  • Regularly appears when you wake up
  • Doesn’t resolve with normal movement
  • Limits your ability to turn your head
  • Keeps returning
  • Interferes with driving, work or exercise
  • Extends into your shoulder or arm
  • Is accompanied by numbness, tingling or weakness

Prompt medical evaluation is appropriate for neck pain associated with significant trauma, fever or serious illness, progressive neurological deficits, or other concerning neurological symptoms.
