Hurt Your Back Lifting Something Heavy? What to Do Next
You bend down to pick up a heavy box, bag of mulch or piece of furniture. As you stand up, you suddenly feel a sharp pain across your lower back.
Sometimes it is immediate. Other times, you finish what you are doing and notice increasing stiffness several hours later.
Lifting-related low-back pain is common, but the fact that the pain started while lifting doesn’t tell us exactly which structure is responsible.
At Backsmart Health Chiropractic in Cumming, Dr. Gregory King evaluates the history, movement and examination findings before recommending treatment.
Why Can Lifting Trigger Low-Back Pain?
Lifting places physical demands on the muscles, joints and other tissues of the lower back.
Risk isn’t determined simply by the number of pounds you pick up.
The National Institute for Occupational Safety and Health lifting guidance considers several factors, including the weight of an object, how far it is held from the body, its starting height, how far it must travel, twisting or asymmetry, frequency and duration of lifting, and how well the object can be gripped. CDC
That helps explain why picking up an awkward 40-pound object can sometimes feel much harder on your back than lifting a heavier object under controlled conditions.
“I Felt Something Grab in My Back”
Patients use different descriptions:
- A sudden grab or catch
- Sharp pain when straightening up
- Muscle tightness or spasm
- Difficulty bending forward
- Pain transitioning from sitting to standing
- An ache concentrated on one side
- Pain that becomes worse later in the day
These symptoms can occur with several different musculoskeletal problems, so symptoms alone shouldn’t be used to diagnose the cause.
Was It Because I Didn’t Lift With My Legs?
Not necessarily.
“Always lift with your legs” is an oversimplification.
How close the object is to your body, whether you’re twisting, the object’s shape, repetition, fatigue and your current physical capacity can all matter.
NIOSH specifically recognizes horizontal distance, vertical position, asymmetry, lifting frequency and duration when assessing occupational lifting risk. CDC
A more practical approach is to keep awkward loads manageable, avoid unnecessary twisting while carrying them, get assistance with loads you cannot comfortably control and change your setup when you’re performing repeated lifting.
Should You Rest in Bed?
For uncomplicated low-back pain, prolonged inactivity generally isn’t the goal.
The World Health Organization emphasizes rehabilitation, self-management and maintaining physical activity as important components of managing nonspecific low-back pain. World Health Organization
That doesn’t mean you should immediately return to heavy deadlifts or move furniture while you’re hurting.
There is a difference between comfortable activity and repeatedly provoking the injury.
Walking and normal daily movement within tolerance may be reasonable while temporarily modifying activities that clearly aggravate the pain.
What If the Pain Goes Into My Leg?
Pain traveling from the lower back or buttock into the leg changes the clinical picture.
Tell your provider if you experience:
- Numbness or tingling
- Burning or electric sensations
- Pain extending below the knee
- Noticeable leg weakness
- Changes in balance
- Symptoms that increase with coughing or straining
These findings don’t automatically mean you have a herniated disc, but they deserve an appropriate evaluation.
When Should Back Pain Be Checked Immediately?
Seek urgent medical assessment when back pain follows significant trauma or occurs with progressive neurological weakness, loss of bowel or bladder control, numbness around the groin/saddle region, fever or other serious systemic symptoms.
Those aren’t situations to simply “wait out.”
Does Every Lifting Injury Need an X-Ray?
No.
Imaging should be determined by the patient’s history and examination rather than automatically performed for every episode of back pain.
At Backsmart, digital X-rays are available when Dr. King determines they are clinically necessary.
Can Chiropractic Care Be Part of Treatment?
For appropriately selected patients, conservative treatment may include education, exercise, manual treatment and activity modification.
The WHO’s chronic primary low-back-pain guideline includes exercise programs and says spinal manipulative therapy may be offered as part of care, while emphasizing an individualized, person-centered approach rather than relying on one treatment in isolation. World Health Organization
Although those WHO recommendations specifically address chronic primary low-back pain, they illustrate an important principle: treatment should be matched to the individual rather than assuming every painful back needs the same intervention.
At Backsmart, Dr. King may use chiropractic adjustments, rehabilitation exercises and other conservative approaches when appropriate after examination. Backsmart Health Chiropractic
How Can I Reduce My Risk the Next Time I Lift?
Before your next heavy lifting project:
Move the object closer when practical. Holding a load farther away increases the physical demands of the task.
Avoid combining heavy lifting with unnecessary twisting. Reposition your feet when possible.
Know when an object needs two people. Awkward furniture and equipment may be difficult to control regardless of strength.
Break repetitive jobs into manageable periods. Fatigue can change how you move.
Build capacity gradually. Strength and physical conditioning matter, particularly when your normal routine involves frequent lifting.
The objective isn’t to become afraid of lifting. It’s to make the demands of the task more appropriate for your current capacity.
